Is there an in-universe reason why his appearance is like that of his 4th form and not his 5th form? When he was being burned by the sun, he was in his 5th form. Perhaps he could've reverted to his original form but the movie never shows this. Toei did this because it would've looked cooler right |

I don't really know to be honest. Perhaps Cooler's final form produces even more strain than the final form of Freeza, so it would be dangerous to force him to maintain that power? When Freeza was turned into his metal form, he wasn't shown to use his 100% power, so it may be the same for Cooler. Going all out in power may shorten his life span because he can't revert back to his suppression forms. |

When Cooler is dying, he's sort of I believe in this transition between forth and fifth. However his true body...head.... in Movie 6 is clearly based upon his fifth form in most regards while his metal bodies are based on his fourth. |

I think Cooler's survival was based on Freeza's. Although, I think there is one change. Due to the last form's strain, he reverts back to his fourth form to have a bigger chance to survive. |

The strain of his final form I guess. That's the reason why Frieza created his supression forms to avoid. |

Another piece of evidence that Coola and Freeza's slim and uncomplicated-looking form are their "true" forms, while their lower-powered ones are suppressive transformations which modify it (and Coola's "super" form was likewise a modification, hence him reverting down from it while succumbing to the sun in Movie 5). So the Metal Coolas would simply be modeled after Coola's most natural state.
