I'd say Joker edges out on being a pure psychotic but Hisoka is certainly more "weird", especially when you factor in his sexual tastes... "Schwing!" These two would probably make great friends.

His main joy in life is fighting, especially if he's fighting someone that might actually beat him or might become powerful in the future. Enjoys fights to the point where it actually turns him on and will groom those with great potential so they'll be a good fight for him later on. His sexual tastes also hold no boundaries. Women? 12 year old boys? Not a problem, so long as they're tough. Quite the bloodlust as well but it's not as strong as his general desire to fight tough foes. He'll also let himself suffer severe injuries just to mess with his opponents mind and throw them off. Here he let's a dude punch both of his arms off in a competition, even though he could easily beat the guy if he actually tried. He has a comrade that can heal his arms but it's not certain whether or not she'd do it since they're not friends by any means, yet he still takes the injuries. Hisoka also gives in to his desires at random which makes him very unpredictable. One second he'll be working with someone and then the next he's willing to risk all his plans to fight to the death with them. Another creepy scene from him: He's an extremely warped individual. Not that I know much about the Joker beyond the norm but he seems a bit more...manic I guess?

I mean, Hisoka sounds insane and all, but he seems more Carnage-ish. Not exactly the same type of insanity as Joker's. Joker just wants the hero to break their one rule, he wants to prove they're no better than him, that we're all crazy and we're all murderers. So if this is a competition of who's crazier, I'd go with Joker, he has a LOOOOOOOOOONG history of crazyness. Plus he once wore The Mask, so he's definetely been into a higher class of insane. And let's not forget about Emperor Joker, who did a lot of awful things. Such as eating everyone in China.
